Nominations can be made by the candidate themselves via self nomination, by a colleague or a third party such as a family from your service. All nominations must be made on an official Children's Services Awards Application Form and accompanied by a Candidate Profile and a Referee Form completed by either a Referee or Nominator. NOMINATIONS CLOSE 28 MARCH 2008.
